The Age-Friendly Health System (AFHS) program was developed by The John A. Hartford Foundation and the Institute for Healthcare Improvement. It emphasizes the “4Ms” approach: What Matters, Medication, Mentation, and Mobility.

Promoting mobility and independence by addressing issues that could hinder an older adult’s ability to move and engage in daily activities.
